Fentanyl changed the stakes: why one counterfeit pill can now kill

October 9, 2026 · 21 min read

Laura Berman lost her 16-year-old son to a suspected fentanyl-laced pill, Stanford's Bonnie Halpern-Felsher warns about fentanyl in the drugs teens experiment with, and former counter-narcotics officer Ed Calderon explains the cartel pipeline that pushes fentanyl into the supply. Together they show that experimenting with a pill bought outside a pharmacy now carries a risk of death that earlier generations did not face, which changes what parents need to tell their kids.

The rules, numbers and steps worth keeping in front of you.
  • Core point: a single counterfeit pill from outside a pharmacy can kill, even the first time.
  • Berman's rule: do not take anything from the street or a dealer. Nobody knows what is in it.
  • The rule covers pills from friends too. In one case Berman describes, a boy in tooth pain died after taking a friend's "Percocet."
  • Counterfeits look like familiar prescription painkillers such as Percocet. According to Calderon, cartel pill presses make them.
  • Halpern-Felsher: many victims were not habitual drug users. They wanted a pill for pain.
  • Dealers reach teens on Snapchat and other apps and may deliver to the home. Berman says banned accounts reappear under new names.
  • Halpern-Felsher's safety layer: no use is best, but if a teen uses, test the pill, never use alone, and have Narcan nearby.
  • Test strips are imperfect. Fentanyl may be in only part of a pill, so a strip can miss it.
  • No one alone can be given Narcan. Halpern-Felsher carries it and wants it in every school, library and bar.
  • Halpern-Felsher: "just say no" and exaggerated warnings fail with teens and cost adults credibility.
  • Talking about drugs doesn't plant ideas. Teens have known about drugs since about age eight.
  • Tell teens you won't bust them, so they come to you for help.
  • Calderon: around 50 to 70 cartel groups and widespread corruption mean the supply won't fix itself.
  • See a pediatrician or a qualified addiction or mental-health professional about your family's situation.
